Baneful Hearts are necessary to summon Blood Maiden, a boss encounter that drops some of the best loot in the game. However, the Baneful Hearts are valuable and not easily found.
How to Farm Baneful Hearts in Diablo IV

The best way to farm Baneful Hearts is by defeating Hellborne enemies. You will get to fight them after becoming HellMarked.
- Defeat Hellborne enemies: For a Hellborne enemy to spawn you need to get HellMarked which you do by raising your Threat levels. To keep it simple, find any Helltide event and start killing monsters there. The more your threat level is the more monsters it spawns and the harder it gets to survive so you will have to move around a lot and take note of your abilities and their cooldown.
- Open Tortured Gift chests: This is the second best way to get Baneful Hearts. You need Aberrant Cinders which you get by defeating mobs of enemies. Open these chests and you can occasionally find a Baneful heart from one of them.
Check the top right side next to the minimap to see how many Baneful Hearts you have. It is above the Threat level meter and next to the Aberrant Cinder symbol.
How to Use Baneful Hearts

After collecting 3 Baneful Hearts, you can place them at Accursed Altars.
- Open the map and look for a devil icon with broken left horn.
- Go there and look for a sigil made between 3 rocks and 3 headless bodies.
- After all 3 Baneful Hearts are used the Blood Maiden spawns which you can fight for rewards.
The good news is you can play with others and don’t have to hunt for all 3 hearts on your own. Make sure you use at least 1 to get good rewards from her fight.
